Clinical Pharmacist Drug Utilization Review
Makes determination on drug coverage based upon medical necessity and compliance
with exception criteria:
• Reviews patient information submitted for medication prior authorization
• Collates clinically relevant documentation to support exception requests for
non-formulary medications
• Utilizes knowledge of standard treatment algorithms for a wide variety of
disease states to:
o Identify medically necessary treatment
o Identify cases where formulary alternatives may be inappropriate
o Identify investigational and/or experimental drug requests
• Supports pharmacy technicians and supervisors with drug requests that require
clinical interpretation and determinations that are outside of printed criteria
• Participates in daily Medical Director rounds to review requests for drugs
that are high cost, newly FDA approved, subject to abuse/misuse or are being
used outside of current standard of care
• Discusses patient specific cases with physicians to make determinations on
drug coverage
Performs medication therapy management for patients by providing the following:
• Obtains necessary assessments of the patient health status and utilize
information obtained to perform a comprehensive medication review that will
identify, resolve and/or prevent medication-related problems
• Formulates and makes recommendations for medication treatment plan
• Monitors and evaluates response to therapy including safety, efficacy and
quality concerns
• Documents medication care delivered and communicates information to other
health care providers involved in patients care
• Provides verbal education and training to enhance patient understanding of
(and adherence to) medication utilization
• Assists the Medication Therapy Management Coordinator in developing clinical
algorithms for disease state management
• Assists in the development of member communications about the medication
therapy management program
Pharm. D. degree in Pharmacy and two years experience in clinical pharmacy plus
current California Pharmacy License or equivalent combination of education and
experience.
Thorough clinical knowledge regarding therapeutic use of drugs.
Excellent oral and written communication skills.
Proficient with computers and related software applications.
Good customer service skills.
Good math, analytical and problem solving skills.
Knowledge base and understanding of multiple pharmacy software systems
Ability to streamline and simplify complex workflow algorithms
